As an integrated acute and community organisation, we have three areas of core business:
Prevention and enablement Community services Planned and emergency hospital care
We plan to embed prevention and enablement in all of our pathways of care, taking advantage of all of our touch points with patients and service users. We want to streamline community and hospital services to provide a seamless service with less reliance on hospital admission.
