As a GP within Derwentside you can use the new Central Referral Management and Booking Service to make direct referrals to the following services:
Adult Speech and Language Therapy This is a countywide service across County Durham and Darlington in both acute and community settings. Staff are based at Darlington Memorial Hospital, Bishop Auckland Hospital, University Hospital of North Durham and Shotley Bridge Hospital in all clinical SLT areas treating patients with:

Podiatry
Nutrition and Dietetics This is a service across the south of County Durham and Darlington in both acute and community settings. Staff are based at Darlington Memorial Hospital, Bishop Auckland Hospital, in all clinical Nutrition and Dietetic areas able to provide assessment and advice for conditions including malnutrition, diabetes, obesity, allergies, gastrointestinal disorders.
To make a referral to any of these services please complete the 'AHP Direct Referral Form' and return it to the team either by email to cdda-tr.centralbooking@nhs.net or by safe haven fax on 01388 825 701.
