County Durham and Darlington NHS Foundation Trust has introduced a new central bookings service for appointments within the following Allied Health Professional Services:  

• Podiatry
• Physiotherapy
• Nutrition and Dietetics
• Adult Speech and Language Therapy

Why has the new system been introduced?

The Trust has introduced a central booking system to manage appointments within these services.The Central Referral Management and Booking Service means that the referral system has been simplified to provide you with just a single point of contact and appointments which are available 52 weeks of the year.

It also means that your patients will benefit from:

• a more streamlined service
• a greater choice of appointment availability, (location, date and time)
• reduced waiting times
• a single point of contact for self referrals

How do I make a referral?

All referrals should now be made by completing the 'AHP Direct Referral Form'.

This form should then be emailed to the central bookings team by email to

cdda-tr.centralbooking@nhs.net or by safe haven fax on 01388 825 701.

Patients are also able to continue to self refer to some of the services and should telephone the central booking team for more information 01388 825 700.
