Reminder: Community Services roadshow evenings, 5.30pm (venues and dates below)

Our clinical strategy includes the commitment to "care closer to home, where it is safe to do so, with services provided within the patient's home where possible, and a sustainable model for community hospitals".

  Our plans for 2015/16 include the introduction of mobile working for community nurses, and the development of new models with primary care, following the introduction of community based multidisciplinary teams in Darlington.

  GPs, practice nurses, practice managers and CCG colleagues are invited to join us for a series of evening roadshows sharing information about the Trust's community services and how, together, we can develop our plans for the future.

  The events will include tours of community hospitals and information displays from services across both adult and children's community services with the opportunity to meet with frontline staff delivering the services and heads of department to discuss any issues or queries.

  The next roadshow is at Chester-le-Street Hospital (pictured, above) on 14 May, then dates as below:

  • adult physiotherapy
  • dietetics
  • health visitors and school nurses
  • midwifery
  • sexual health
  • intermediate care
  • safeguarding
  • speech and language
  • family nurse partnership

  6.00pm: Buffet

6.30pm - 8pm: Executive directors present an update on the Trust's clinical strategy and plans for the future with an opportunity for Q & As at the end.
