Section 29 Police requests for information held by County Durham and Darlington NHS Foundation Trust

 

Medical statements

To request medical statements from Trust staff, a patient consent form must be completed by the Police / local authority or law enforcement agency and signed by the patient. This should then be submitted to the Police request inbox: cdda-tr.Policerequests@nhs.net

Medical notes and patient related documentation

To request medical notes  / x-rays / scans / A&E records or any other patient medical documentation from the Trust, a Section 29 form must be completed by the Police / local authority or law enforcement agency, fully stating what information is required, the reason the information is required and why it is being requested.

It must be signed by a Police Sergeant or above or a senior manager in other agencies, and is subject to verification of that person's status. Where medical notes are requested, a signed patient consent form must also be completed fully and submitted with the section 29 form to cdda-tr.Policerequests@nhs.net
