The community rehabilitation team
consists of qualified physiotherapists and occupational therapists
supported by therapy assistants. All our qualified staff are

We work with people aged 18 years and over who have a wide variety
of physical illness and / or disability eg stroke, mobility
problems, Parkinson's Disease, falls, amputation, orthopaedic

Where is the service provided
There are currently 2 community rehabilitation teams working within
the north of the County.
Durham and Chester le Street team is based at Chester le Street
Derwentside team is based at Shotley Bridge
When is the service provided?
The service is delivered 5 days per week from Monday to Friday and
is available between 8.30 - 4.30 each day.
How do people access the service?
Referrals are received from:
a) Community services ( district nurses, GPs, community
matrons, mobile warden services, social workers etc).
b) Hospital therapy teams to continue post discharge
