Referring to Occupational Therapy

We have an open referral system which means that anyone who knows or works with a child or young person registered with a GP or living within the three Clinical Commissioning Groups of County Durham and Darlington can refer to our Occupational Therapy Service. This includes parents/carers, teachers, other health professionals, consultants, GPs.

We do ask that if a professional is referring, they obtain consent from the parent/carer before the referral is sent.

To refer to our service you can call the Central Helpdesk or download our referral form below. Completed referral forms are to be returned to our central helpdesk via post, email or the clinical computer system - SystemOne.

The Sunflower Program:

Sunflower

Families in Darlington wiith primary aged school children will be able to access The Sunflower Program. Further information can be found in the Sunflower Program Information Leaflet. This service will run for one year from September 2022 until August 2023. Referrals can be made by the school the child attends or professionals involved with the family or child by completing the following Referral Form:
