Pharmacy and Medicines Management

The Pharmacy and Medicines Management team provide services to a range of acute and community wards and departments. These services include:

  • Medicines reconciliation service
  • Ward top-up service, dispensing and distribution of medication
  • Aseptic chemotherapy service
  • Antimicrobial stewardship
  • Education and training for medical and nursing staff
  • Analysis of medication incidents drug information.
  • Lead on safe medication practice, safe and secure handling of medicines and provide controlled drug assurance to County Durham & Darlington Foundation Trust
