The Community Falls Service supports people aged 65 and over to reduce their risk of falling through specialist assessment, personalised advice and targeted support.

Delivered countywide, the service focuses on promoting independence, improving confidence, and preventing further falls.

John Fife, aged 81, was referred to the service following a series of falls that were affecting his safety and daily life.

After a comprehensive assessment, the Community Falls Service worked with John to understand the possible causes of his falls and develop a tailored plan to support his recovery.

As part of this plan, John was provided with home adaptations and equipment to help him manage daily activities more safely.

Once the medical aspects of his condition had been addressed, he began a structured strength and balance exercise programme, designed specifically around his needs and abilities.

Following surgery, John’s programme was carefully reviewed and progressed. His rehabilitation started with home-based exercise sessions, before moving on to a combination of group strength and balance classes at Sedgefield Community Hospital.

This gradual progression helped John rebuild confidence, improve mobility, and continue his recovery in a supportive environment.

John’s care was supported by Ana Etchells, Senior Specialist Physiotherapist, whose expertise played a key role in guiding his rehabilitation and ensuring his programme remained safe, effective and person-centred.

The Community Falls Service provides high-quality therapeutic interventions aimed at achieving realistic, patient-centred goals. The team works closely with patients, families and carers, and collaborates with other health and social care agencies to promote independence in daily life.
