At the beginning of April, Professor Richard Scothon, Chairman of the Trust, visited the Health Squad at the Greenhouse to witness firsthand the innovative work being carried out to improve health outcomes for individuals in our community.
During his visit, Prof Scothon was deeply impressed by the team's commitment to championing a culture of change - a vital shift needed in healthcare to address inequitable outcomes, particularly within the core 20 plus population identified by NHS England.
This population includes individuals who face the most significant health challenges and barriers to accessing care.
The Health Squad provides a range of health interventions directly to individuals who might otherwise miss out on essential services.
These interventions include full NHS health assessments, cholesterol checks, smoking cessation, spirometry, and asthma care.
By taking services directly to people in need, the team ensures that healthcare is accessible, particularly to those who might face barriers due to social or economic factors.
The Health Squad also plays a critical role in supporting individuals to navigate both physical and mental health systems.
They offer signposting and referrals to the most appropriate services, ensuring that patients receive the care they need at the right time.
By working closely with commissioned, statutory, and voluntary organisations, the Health Squad delivers collaborative, patient-centred care that leads to positive outcomes for the individuals they support.
The team's core belief is that every individual deserves access to quality healthcare, and they are passionate about providing unwavering support to those who have faced significant adversity.
Their vision is to guide individuals toward transformative lifestyle choices that promote lasting wellbeing, resilience, and a brighter future.
The visit from Prof Scothon sparked meaningful discussions, with the team feeling encouraged by his support and recognition of the essential work they are doing to challenge health inequities and provide holistic care to some of the most vulnerable in our communities.
The Health Squad continues to make a tangible difference in people’s lives, and the team's innovative approach stands as a shining example of how healthcare can be more inclusive, accessible, and effective in tackling health disparities.
