If you’ve been identified as at a higher risk of developing preventable health problems such as heart disease, diabetes, kidney disease or stroke. The Health Checks team will contact you to discuss ways they can support you to make lifestyle changes to reduce that risk.
If you’re aged 40 to 74 and do not have a pre-existing health condition, you should be invited to an NHS Health Check by your GP or local council every five years.
An NHS Health Check aims to reduce the instances of cardiovascular disease (CVD) and prevent heart disease, stroke, kidney disease and diabetes and in some cases, dementia.
If you think you are eligible but have not been invited, contact your GP surgery. If you are offered an NHS Health Check, it is important that you make an appointment. Identifying risks and making lifestyle changes earlier could save your life.
Our friendly team can signpost you to local health and wellbeing services where you will receive the support you need to make changes which will benefit you long-term.
