Adult Wellbeing Services (AWS) is a range of services which work together to reduce health inequalities and increase opportunities for individuals and communities across County Durham.
The County Durham Approach to Wellbeing underpins everything we do and is based on improving people’s wellbeing through implementing community centred approaches. The Approach to Wellbeing is about putting people at the heart of everything we do.
Commissioned by County Durham Council, Adult Wellbeing Services is a partnership between our Trust and the Pioneering Care Partnership (PCP) which delivers wellbeing and lifelong learning support within the community. A new 10-year contract was awarded in late 2023 to continue to provide services, including:
Joining the Dots
Joining the Dots supports people aged 18 and over to live well when they have received a cancer diagnosis, are going through cancer treatment, or are living beyond their diagnosis.
Joining the Dots Facilitators are also there to support immediate carers so that they can support the person with the cancer diagnosis.
We can help with anything from making small changes to support a healthy lifestyle, to helping people get the right benefits through Macmillan Welfare Rights.
