Colleagues in the Frailty Assessment Service at Darlington Memorial Hospital have received glowing praise from a local GP for the support they provide to patients and primary care colleagues.
Dr Shashikiran G Chandrasekhar, from Pinfold Medical Practice in Bishop Auckland, wrote to County Durham and Darlington NHS Foundation Trust Chief Executive Steve Russell to personally thank the team for the outstanding support and service they provide.
Dr Chandrasekhar highlighted the difference the service has made to his patients, particularly by helping them avoid long waits in hospital and supporting the management of complex health needs in primary care.
In his letter, he said: “I would like to convey my thanks to the Frailty Assessment Service led by the Geriatrician of the Day at Darlington Memorial Hospital who have been exemplary.
“I have used them on a number of occasions and have found them extremely helpful and dedicated. They have helped my patients avoid long waits in the hospital and help manage complex issues in primary care.
“Kudos to the service and the team.”
The Frailty Assessment Service provides specialist assessment and support for older people living with frailty, helping patients receive the right care and advice while supporting colleagues in the community.
Feedback such as this highlights the important role the service plays in supporting patients and strengthening partnership working between hospital and primary care teams.
