Ensuring your individual needs are met during your time in hospital

Hospital Passports, also known sometimes as Health Passports, are documents you can fill out, to provide hospitals and other health and social care settings with important information about yourself, a friend or family member. 

Hospital passports provide key pieces of information in a simple informative way to those caring for you, to ensure all of your individual needs are met during your time in hospital, It can help you to get the personalised care you need. 

Bring your hospital passport with you

You should bring your hospital passport with you to all appointments and keep it with you during overnight stays or visits to the Emergency Department. You should show it to healthcare staff as soon as you can when you arrive at hospital.

Below you will find links for hospital passports currently used across County Durham and Darlington NHS Trust hospitals, however, if you already have a passport from another hospital or area please be assured you can also use this - this is your passport and we want to support you and meet all of your needs.
